: Human research ethics committee approval was obtained at each of the centres participating to the ANZCOSS and VANESSA cohorts (Victoria, Australia: Austin Health, Alfred Health, Southern Health, Barwon Health; New South Wales, Australia: Liverpool Hospital, Royal Prince Alfred Hospital, Westmead Hospital; Queensland, Australia: Princess Alexandra Hospital; West Australia: Royal Perth Hospital; Tasmania, Australia: Royal Hobart Hospital; Auckland, New Zealand: Auckland District Health Board). Written informed consent was obtained from participants to the prospectively collected VANESSA cohort. Consent from each patient for ANZCOSS was not required as the collection of data was considered to be a clinical audit. Research was performed in accordance with the Declaration of Helsinki.
