Ethics approval and consent to participate
: The study was centrally approved by the North of Scotland Research Ethics Service (Reference no. 13/NS/0155) for all 33 UK centres and will be conducted according to the principles of Good Clinical Practice provided by Research Governance Guidelines. Annual progress reports and a final report at the conclusion of the trial will be submitted to North of Scotland Research Ethics Committee 2 within the timelines defined in the regulations.
