: All study procedures were approved by the Institutional Review Board (IRB) at the University of North Carolina at Chapel Hill and the Kenya Medical Research Institute Ethical Review Committee. All participants provided verbal consent to participate in the study. Verbal consent, rather than written consent, was considered appropriate for this study by each of the in-country ethics committees because it is the standard procedure used for these types of large-scale demographic surveys.
