: All participants were informed about the purpose and activities of the evaluation in written material in which we provided contact details for the research team. All workshops and individual discussions were held in locations, dates and times convenient to the participants. Participants were free to leave the process at any time, and refreshments were provided during the workshops. Institutional review boards at the University of the Witwatersrand (M1704155; M171050) and the University of Aberdeen (CERB/2017/4/1457; CERB/2017/9/1518) reviewed and approved the VAPAR study protocols, and the provincial health authority gave permission for the research (MP_201712_003). Electronic data was stored on a secure server hosted by the University of Aberdeen and automatically backed up daily.
