: All participants were included after written informed consent, and none had chosen to withdraw from the study at the time of analysis. The study has been approved by the Capital Region regional ethics committee (H-16001790) and the Danish Data Protection Agency (HGH-2016-017). The study was also registered at ExternalRef removed (NCT02772549). We conducted the study according to the principles of the Declaration of Helsinki and adhered to the STROBE guidelines.
