Ethics approval and consent to participate
: Evaluation of the effectiveness of the intervention is approved by the Danish Data Protection Agency (J.nr. 2012–41-0763) and by the National Health Board (3–3013-608/1/). The regional ethics committee confirmed that approval of the study was not required because biomedical experiments were not included (J. nr.1–10–72-108-14). All patients in the intervention group will receive written and verbal information about the conditions of their participation and sign informed consent prior to inclusion. The study is registered in ExternalRef removed (NCT02078466). The pilot study was approved by the Danish Data Protection Agency (J.nr. 2012–41-0763).
