Ethics approval and consent to participate
: The UCL Research Ethics Committee granted approval for the focus groups and all participants gave written consent (see Additional file InternalRef removed: Appendix 3). The same committee advised that as the survey was anonymous, voluntary, and had no financial reward for completion, formal ethics approval was not required.
