: Following the initial searches, a decision was made to focus only on published literature and therefore grey literature was not included. The decision was made after the protocol was published, which is therefore a deviation from the protocol. However, it was felt that searching both medical and psychological databases and journals would offer the greatest likelihood of returning papers relevant to the research question whilst remaining practicable within the limits available to this PhD. A decision was also made to exclude all intervention studies, regardless of whether prevalence data on factors associated with psychosocial functioning could be derived (e.g., where quality of life was measured at baseline). This decision was made after the protocol was published; however, it was felt that as intervention efficacy was not being explored, this exclusion criteria would enable the authors to best address the research objectives.
