Ethics approval and consent to participate
: All procedures will be carried out in compliance with the Helsinki Declaration. The study was approved by the Medical Ethics Committee (METC) at VU Medical Centre Amsterdam (protocol. nr. 2016.162, see Additional fileInternalRef removed). Before the start of the study, written informed consent will be obtained from each patient and one of their parents (or guardians). All information about the participants will be kept strictly confidential. Clinicians will inform the patients and their parents that participation in the study is entirely voluntary and that, if they refuse, their decision will not affect the care as usual they receive. All patients will also be informed of their right to withdraw from the trial whenever they desire without giving the researchers any reason for their decision.
