The participants will receive the standard professional health care at both clinics, and to the best of our knowledge, there are no risks associated with participation in the study. The participants will sign a written informed consent form and will be free to withdraw their consent at any time. The Regional Committee for Medical and Health Research Ethics in Western Norway approved this study (No.: 2011/934). The trial is registered in the ISRCTN registry (No.: 12033424).
