: Approval to conduct this study was obtained from the Makerere University School of Medicine Research Ethics Committee (<i>2020–133</i>) and Uganda National Council for Science and Technology (HS845ES). Administrative clearance was obtained from the Director of Health Services KCCA. Informed written consent was obtained from all participants; confidentiality and anonymity were strictly observed at all the research stages. For illiterate participants, informed consent was obtained from participants and in presence of an impartial literate witness who was either their guardian or another literate person who was not part of study team and it approved by the Institutional Review Board of Makerere University School of Medicine Research Ethics Committee (<i>2020–133</i>).
