: The MCS secured ethical approval from the National Health Service Multi-Centre Research Ethics Committee for each sweep (see ExternalRef removed). Written informed consent was required and obtained from parents/legal guardians. At the age 14 follow-up, children also provided informed consent. This study required no further ethical approval for secondary data analysis. All methods comply with the relevant national and institutional committees on human experimentation, including the ethical standards outlined in the 1975 Helsinki Declaration, as revised in 2013.
