Ethics approval and consent to participate
: The study protocol was approved by the Ethics Review Committee of Sri Lanka Medical Association and the Ethics Committee of the National Centre for Global Health and Medicine, Japan. Written informed consent was obtained from the adults who agreed to participate in the study. We obtained informed verbal consent from all participants of youth clubs who participated in this study. In the event of children below 18 years who were members of a youth club, additional verbal consent was sought from their parents or guardians. Since we did not collect individual information from youth club members, the Ethics Board approved that we received simply verbal consent from their youth and parents when required. The study was registered in the Sri Lanka Clinical Trials Registry (SLCTR/2017/002). Registered on 19th January 2017, ExternalRef removed, Date of enrolment of the first participant to the trial was 1st February 2017.
