: The study was conducted in accordance with relevant guidelines and regulations in the Declaration of Helsinki. Since this study was based on register data, ethical approval including informed consent was not required according to Danish legislation and the National Research Ethics Committee. In accordance with Danish law and the EU’s General Data Protection Regulation, the project was listed at the Aarhus University internal list of research projects (J. no.: 2016-051-000001, sequential no.: 1192). A data processing agreement was contracted between Aarhus University and Statistics Denmark, and Research agreements and Affiliation agreements were contracted between Statistics Denmark and both SB and MI. A cooperation agreement between Aarhus University and Odense University Hospital was furthermore entered. By means of the registration at Aarhus University and all mentioned agreements, approval for receiving individual-based data with encrypted CPR numbers from all those used registers (DCR, DCRS, DNPR, and DQCCS) was given by the Danish Health Data Authority. All data were transferred from the Danish Health Data Authority’s research machine to the research server at Statistics Denmark (i.e. the ‘data processor’) with Aarhus University registered as ‘data responsible’.
