Ethics approval and consent to participate
: The study (Ref. No. 2015-Apr-001) was approved by Excellence in Research Ethics and Science Converge (ERES Converge) ethical review board (I.R.B. No. 00005948 F.W.A. No. 00011697) in Lusaka. Permission to conduct this research in the health facilities was also sought from the Ministry of Health of Zambia. Written informed consent and assent were obtained for surveys, key informant interviews and focus group discussions and all efforts were made to keep respondents’ information confidential. Since the target age group was 15–49 (reproductive age group), for participants below the age of 16 the intention was to obtain written assent from the guardian or parent. However, in the final random sample no one under the age of 16 was selected. There was no known risk or immediate benefit to participate in this study.
