Ethics approval and consent to participate
: The study was approved by the Ethics committee of the University Medical Centre Leiden and the boards of the six participating hospitals: Onze Lieve Vrouwen Gasthuis Amsterdam, VU Medical Centre Amsterdam, Academic Medical Centre Amsterdam, Lucas Andreas Hospital Amsterdam, St. Antonius hospital Nieuwegein, Diakonessen hospital Utrecht (ref. no. P10.240).Prior to participation in the RAVEL study, women were asked to read and sign a consent form which informed them of the purpose of the study and they were told that that participation was voluntary. Antenatal and natal care was provided to all women regardless of whether they consented or declined to participate in the study. All participants signed the informed consent form. Parental consent was not necessary, all participants were at least 18 years. All data were used anonymised.
