: The Human Research Ethics Committee of The Aga Khan University (3612-CHS-ERC-15) and Dow University of Health Sciences Karachi Pakistan (IRB-557/DUHS/APPROVAL/2015/84) approved this study. The purpose and the procedure of the study were explained to the participants before written informed consent was obtained. Then written informed consent was obtained from study participants who can read and write. However, those participants who were not able to write, a thumb impression was taken. Moreover, the written informed consent was also obtained from the CBO administration person from where the participants were recruited. The study participants were made assured that their provided information would remained be confidential. Moreover, all the methods were performed in accordance with the relevant guidelines.
