: Ethics clearance was obtained from the Biomedical Ethics Committee at the University of Zambia,Lusaka, Zambia; the Stellenbosch Health Research Ethics Committee, Tygerberg, South Africa; and the London School of Hygiene & Tropical Medicine Ethics Committee, London, UK. Governmental approval for the trial was obtained in both countries. All individuals involved in the study gave written informed consent.
