: Ethical approval for the study protocol was provided by the Keio University Faculty of Pharmacy Ethics Committee for Research Involving Humans (No. 160118–1), in accordance with local ethical guidance for medical research involving human subjects. Informed consent of each patient was waived by the committees, because this study used anonymized data which was retrospectively collected from medical records at the responded medical institutes.
