Ethics approval and consent to participate
: The research proposal has been reviewed by the medical ethics committee of the Erasmus Medical Centre in Rotterdam, the Netherlands (study protocol number MEC-2014-444). The committee decided that the rules laid down in the Medical Research Involving Human Subjects Act did not apply. Consent to participate in the evaluation study was obtained from all GP practices. In our theory-guided study protocol we do not report on any data relating to individual participants. Informed consent to participate in the study is obtained from all patients. Verbal informed consent to participate in the face-to-face interviews is obtained from all GPs. With respect to the research involving healthcare professionals, we explain in a letter that consent is implied upon return of a completed questionnaire.
