Ethics approval and consent to participate
: All procedures performed in studies involving human participants were in accordance with the ethical standards of the institutional and/or national research committee and with the 1964 Helsinki declaration and its later amendments or comparable ethical standards. The study was approved by the Regional Ethical Review Board in Umeå, dep for medical research, (Regionala etikprövningsnämnden i Umeå avdelningen för medicinsk forskning); EPN 2015/186–31 and EPN 2016/502–32. In Sweden the Regional ethical boards used to be a part of the faculty of medicine of the regional University until 2004. Between 2004 and 2019 the Regional ethical boards were independent authorities. From 1 January 2019, applications for ethical examination of research are scrutinized by the new Swedish Ethical Review Authority.Informed consent was obtained from all individual participants included in the study. The respondents were informed about the research project and that completion of the web-based survey was accepted as consent.
